Staffordshire Crisis & Resilience Fund - 1-to-1 Support Request Form

Eligibility Criteria:

The individual needing financial support meets one or more of the following criteria.

• Lives in Staffordshire and can demonstrate proof of residency*.
• Is a Staffordshire care Leaver.
• Is a vulnerable adult, or adult experiencing financial crisis**.
• Is an adult caring for vulnerable person(s) including children and young people.

If you believe you may be eligible for Crisis and Resilience support, but are not working with a professional, please complete the following form to request an appointment with a 1‑to‑1 support coach, who will asses your need.

South Staffordshire

Stafford Borough

Staffordshire Moorlands

Tamworth
